椎动脉型颈椎病的术式选择

目的　探讨椎动脉型颈椎病的术式选择。方法　采用五种手术方式治疗椎动脉型颈椎病 2 2例 ,其中横突孔切开减压 2例 ,横突孔切开减压、钩椎关节切除 6例 ,颈椎间盘切除、植骨融合术 9例 ,颈椎植骨融合加钩椎关节切除 2例 ,颈椎管扩大成形术 3例。结果　随访 1年～ 3年 2个月 ,优 9例 ,良 7例 ,可 4例 ,优良率 80 %。结论　椎动脉型颈椎病并非单纯由椎动脉受压所致 ,针对不同的原因选择手术方式 ,可以取得良好的治疗效果

Surgical Selection for Treating Cervical Spondylosis of Vertebral Ar terial Insufficiency Type

Objective To analysis the surgical selection for treating thecervical spondylosis of vertebral arterial insufficiency type. Methods Twenty two cases were operated with five different operatio ns: 2 were incision of the transverse process, 6 were incision of the transverse process plus resection of the Luschkas joint, 9 were cervical disce tomy and fusion, 2 were cervical fusion plus resection of the Luschka 'sjoint, 3 were laminopasty.Results Twenty cases were fol lowed -up from 12 to 38 months, the excellent and good rate were 80 percent.Conclusion The pressure of vertebral artery was not the sole reason causin g cervical spondylosis of vertebral arterial insuffciency type. Selecting appropriate operationes according to different cases will achieve good results.
