心脏位置异常复杂畸形的超声心动图与心血管造影对比研究

摘    要:目的:比较经胸超声心动图(TTE)与X线心血管造影(CAG)对先天性右位心合并复杂畸形的诊断价值。方法:对65例先天性右位心合并复杂畸形患者术前的TTE及CAG诊断结果进行对比研究,并与手术结果相对照,评价两种诊断方法对右位心各节段连接及合并畸形的诊断符合率及差异性。结果:TTE和CAG对右位心各节段的诊断以及心内和心脏大血管连接部畸形诊断符合率均在89%以上,两者经统计分析无显著性差异;在大血管畸形的诊断上两者有非常显著性差异(P<0.001),CAG诊断符合率高于TTE。结论:TTE和CAG均能准确进行先天性右位心各节段及合并复杂畸形的诊断,TTE是无创诊断合并复杂畸形右位心的最基本和有效的方法,但在大血管畸形诊断方面,CAG仍有其优势。

Abstract:Objective: To compare the clinical diagnostic value of transthoracic echocardiography (TTE) and angiocardiography (CAG) on congenital dextrocardia with complex cardiovascular anomalies. Methods: TTE and CAG were retrospectively reviewed in 65 patients with congenital dextrocardia with complex cardiovascular anomalies. Their coincidence rates in segmental connection and anomalies between them were assessed. Cardiac surgery results served as "golden standard". Results: The coincidence rates of both TTE and CAG in diagnosis of segmental connection, cardiac anomalies were over 89%. There was no statistical significance between them (p >0. 05) , There was statistical significance in diagnosing vascular anomalies (p <0. 001) ,CAG had a higher coincidence rate than TTE. Conclusions: Both TTE and CAG could make an accurate diagnosis of congenital dextrocardia combined with complex anomalies. TTE is a reliable noninvasive technique. However, CAG has superiority in diagnosing vascular anomalies.
