血液成分在严重烧伤患者休克期急救中的应用

目的总结成都市"6.5"公交车燃烧事件严重烧伤患者休克早期急救中血液成分的应用情况。方法对本院接治的"6.5"事件烧伤患者中的16名危重患者,在休克期第1个24h共输入新鲜血浆2.82万ml,平均(1763±1249)ml(0—5000ml),未输入红细胞,血小板及全血;第2个24h共输入新鲜血浆2.58万ml,平均(1613±1097)ml(0—4800ml),2人输入红细胞15U,未输入血小板及全血。结果16例烧伤危重病例中,除1名患者在休克期第2个24h后死亡外,其余患者在输注血液成分后均平稳度过休克期,救治成功,现正在康复中。结论严重烧伤患者早期除应大量补充晶体及其它胶体液外,输注血浆及红细胞等血液成分也尤为重要。

Blood component transfusion in the treatment of early postburn shock

Objective To summarize the transfusion of blood components in the first aid of early postburn shock in a group of severe burned patients of "6.5" Chengdu bus fire emergency events.Methods Blood component transfusion was observed in the first aid of the 16 severely burned patients. In the first 24h shock period,28 200 ml fresh plasma were transfused to 16 patients,with an average of (1 763±1 249) ml per patient,and no red blood cells,platelets or whole blood were transfused. In the second 24h shock period,25 800 ml fresh plasma were transfused,averaging (1 613±1 097) ml per patient,platelets and whole blood were not transfused. Two patients received 15 U red blood cells.Results One patient died in the shock stage,and 15 cases of severe burned patients remained stable through the shock period and right now are in the recovery. Conclusion In the first aid of severe burned patients,it is important to transfuse plasma,red blood cells and other blood components,besides sufficient colloids to the patients.
