硫酸鱼精蛋白注射液过敏致死1例

1例26a女性患者,因先天性室间隔缺损、二尖瓣关闭不全行室间隔缺损修补术,术中给予硫酸鱼精蛋白注射液220mg中和肝素,10min后患者出现肺动脉痉挛、右心系统膨胀、血压下降、心率减慢、继之心跳停止。立即停药进行心内按压,并给予皮质激素、血管活性药物等治疗,再次紧急体外循环辅助,经9h体外循环支持,患者心功能仍不能恢复,最后死亡。

One case report of allergy to death caused by protamine sulfate injection

A 26-year-old female patient with congenital ventricular septal defect and mitral incompetence received repair of ventricular septal defect.And 220 mg of protamine sulfate injection was administered for neutralization of heparin.Ten minutes later,the patient showed spasm of pulmonary artery,expansion of cor dextrum,reduction of blood pressure and decreased heart rate,and later cardiac arrest.Discontinued the protamine sulfate injection,pressed the heart for cardiopulmonary resuscitation and gave corticosteroids,vasoactive drugs immediately.After 9 hours for urgent extracorporeal circulation once again,the patient＇s cardiac function still could not be restored and she died eventually.
