喷他佐辛用于腹腔镜胆囊切除术后患者自控静脉镇痛的临床观察

目的 观察喷他佐辛用于腹腔镜胆囊切除术后患者自控静脉镇痛（PCIA）的效果。方法选择择期行腹腔镜胆囊切除术60例，随机分为喷他佐辛（P）组和芬太尼（F）组，每组30例。PCIA配比为：P组喷他佐辛（北京双鹤药业生产）150mg加生理盐水，泵总量100ml，F组芬太尼1．0mg加生理盐水至100ml。两组负荷量为2ml，持续输注速度2ml／h，单次追加剂量1ml，锁定时间15min。观察术后4、8、12、24、48h各时段镇痛评分（VAS）、镇静评分（Ramsay）和不良反应。结果两组患者术后镇痛及镇静评分均满意，差异无统计学意义（P〉0．05）；P组恶心、呕吐发生率明显低于F组（P〈0．05）。结论喷他佐辛用于腹腔镜胆囊切除术后患者自控静脉镇痛，效果确切，不良反应少。

Efficacy of Pentazocine for Patient-Controlled Intravenous Analgesia in Patients Undergoing Laparoscopic Cholecystectomy

Objective To observe the effect of pentazocine for patient - controlled intravenous analgesia （PCIA） in patients undergoing laparoscopic cholecystectomy. Methods Sixty patients scheduled to undergo laparoscopic cbolecystectomy,were randomly divided into two groups ： pentazocine PCIA group （ group P） and fentanyl PCIA group （group F）. The drugs used were pentazocine 150 mg in group P,or fentanyl 1.0 mg in group F. The drugs were diluted with normal saline to 100 ml. The settings of patient -controlled intravenous analgesia were as follows：initial loading volume 2 ml background infusion 2 ml/h,bolus volume 1 ml, and lockout interval 15 min. The analgesic effects were assessed by Visual Analogue Scale （VAS） and Ramsay score at 4,8,12,24 and 48 hour after operation,and the adverse effects of nausea,vomiting and some others were recorded. Results There were no differences of VAS and Ramsay score in the patients between both groups （ P 〉 0.05 ）. The incidence of postoperative nausea and vomiting （PONV） was lower in group P than in group F （ P 〈 0.05 ）. Conclusion Pentazocine produced equivalent analgesia for patient controlled intravenous analgesia in patients undergoing laparoscopic cholecystectomy, compared with fentanyl ,and pentazocine produced less side effects.
