中央带前列腺癌的MR扩散加权成像诊断价值

摘    要:目的 分析中央带前列腺癌的MR扩散加权成像(DWI)和ADC图表现,探讨其鉴别前列腺癌与前列腺增生的价值.方法 包括15例中央带前列腺癌及20例前列腺增生.所有病例行MR DWI扫描,b值为800 s/mm2.分析各病例的DWI和ADC图表现,并分别测量癌区、前列腺增生组织的ADC值,统计分析两组间是否存在差异.结果 15例中央带前列腺癌在DWI图像上均呈明显高信号,相应ADC图呈低信号,能直观显示肿瘤的范围.其中6例肿瘤局限在中央带,常规MRI诊断为前列腺增生.前列腺癌组织的平均ADC值为(0.93 ± 0.13)×10-3 mm2/s,前列腺增生组织的平均ADC值为(1.53 ± 0.12)×10-3 mm2/s,两者之间差异有统计学意义(P<0.05).结论 DWI信号特点以及ADC值对中央带前列腺癌与前列腺增生具有较高的鉴别诊断价值,可提高前列腺癌的诊断符合率.

Abstract:Objective To discuss the diagnostic value of diffusion weighted imaging (DWI) in prostate cancer of central glands (CGPca) and differentiate from benign hyperlasia (BHP). Methods All patients included 15 cases of prostate cancer of central glands and 20 eases of benign prostatic hyperplasia. DWI were obtained with factor b of 800s/mm2. DWI and ADC map ap- pearances were reviewed in all patients. ADC values of CGPca and PBH tissues were measured and analysed statistically. Results All of 15 CGPca cases showed markedly high-signal on DWI and low-signal on ADC map, both could clearly demonstrate the area of CGPca. But 6 cases were misdiagnosed as PBH by conventional imaging because of the tumors limited to central glands. The mean ADC values of CGPca cases and BPH cases were (0.93 _+ 0.13) x 10-3mm2/s and (11.53 + 0.12) x 10-3mm2/s respectively. There were significant differences in CGPca and BPH. Conclusion It has an important clinical value in diagnosing prostate cancer of central glands and differentiating from BHP with DWI and ADC values.
