家庭病床患者的功能状态和心理状态及家庭功能状况调查

目的研究家庭病床患者的功能状态、家庭功能和心理状态。方法对238例家庭病床患者应用自制基本情况调查表、COOP/WONCA功能状态量表、90项症状自评量表(SCL-90)、家庭关怀度指数(APGAR)问卷对患者的功能状态、家庭功能和心理状态进行评价。结果70岁以上的老人所占比例较大。只有10.1%的患者总体功能良好。患者躯体化、抑郁、恐怖、强迫症状、人际敏感、敌对、偏执因子分与常模比较,差别有统计学意义(P<0.05)。患者对家庭功能评价良好者占63.9%,只有8.0%的患者认为自己的家庭功能存在严重障碍。家庭功能评分与SCL-90总分呈正相关(P<0.05);COOP/WONCA中生理适应性、情感、健康变化、整体健康评分与SDL-90总分呈正相关(P<0.05)。结论家庭病床患者以老年人为主,总体功能较差,且存在一些心理问题;家庭功能与心理状态相关,心理状态又与个人功能状态相关。

Investigation of Functional State and Psychological State and Family Function for Patients in Hospital Bed at Home

Objective To investigate the functional state and family function and psychological state for patients in hospital bed at home.Methods The data of 238 patients in hospital bed at home about their functional state and family function and psychological state were surveyed by using COOP/WONCA scale and APGAR scale and SCL-90 scale.Results The elderly more than 70 accounted for the large ratio.Only 10.1 of the patients had good general function.The scores in somatization,depression and affright were obviously higher than those of the controls,with a significant difference(P<0.05).The patients who had a good assessment on their family function accounted for 63.9 %,only 8.0 % of the patients believed there was a serious disorder of their family function.The score in APGAR scale was positively correlated with that in SCL-90 scale(P<0.05).The scores of physiological adaptation,emotion,health change and general health in COOP/WONCA scale were positively correlated with the total score in SCL-90 scale(P<0.05).Conclusion The patients in hospital bed at home are mainly elderly persons,and their general function is poor and they have some psychological problems.The family function is related with psychological state that is in turn related with individual functional state.
