大鼠原位肝移植二袖套法模型的手术体会

目的对大鼠原位肝移植二袖套法模型建立过程中的手术进行改进。方法在原袖套法的基础上,将供肝的膈肌环与受体的肝上下腔静脉血管壁一针连续直接吻合;在受体门静脉及肝下下腔静脉吻合前放出部分高凝血液,以减少血栓形成。结果大鼠原位肝移植二袖套法模型建立时血管吻合切实可靠,手术成功率达90%,大鼠1周存活率87%。结论该模型稳定可靠,熟练的显微外科技术、精细的手术操作和极大的耐心是手术成功的关键。

An improved two-cuff technique for orthotopic liver transplantation in rats

Objective To modify the conventional two-cuff technique in the orthotopic liver transplantation(OLT) in rats.Methods During the operation of OLT in rats by two-cuff technique,a continuous stitching was used in the anastomosis of the recipient′s SVC(suprahepatic vena cava) and the donor′s Phrenic ring;some hypercoagulable blood was released from the PV(portal vein) and IVC(infrahepatic vena cava) of the(recipient′s) liver.The donor′s PV and IVC were skeletonized when they were in abdomen.Results The improved anastomosis method of SVC was effective,for 90 percent resulted in successful operation and 87 percent of the rats survived over one week.Conclusion This model is a effective measure to study the liver transplantation in rats.However,rich experience in microsurgery and careful handling in operation can be the key to successful operation.
