糖尿病视网膜病变治疗过程中视力损害原因分析

目的 探讨引起糖尿病视网膜病变患者视力损害的原因。方法 对接受激光光凝和玻璃体切割治疗的１６０例（１９８只眼）糖悄病视网膜病变患者的随访视力进行比较，并对与视力损害有关的因素进行分析。结果 １６０例（１９８只眼）糖尿病视网膜病变患眼中８９只眼（４４．９５％）发生严重视力下降，在发生视力损害的病例中平均血红蛋白和胆固醇明显高于无视力损害病例（Ｐ〈０．０１和Ｐ〈０．００１）。视力损害的发生与玻璃体出血、黄斑水肿和黄斑区色素上皮改变损害有关（Ｐ〈０．０１和Ｐ〈０．００１）。视力损害的发生与玻璃体出血、黄斑水肿和黄斑区色素上皮改变损害有关（Ｐ〈０．０１）。结论 糖尿病视网膜病变治疗过程中可以发生永久性视力损害。视力损害发生与玻璃体出血、黄斑水肿、黄斑区色素上皮损害、高血红蛋白和高胆固醇等因素有关。

An analysis on cause of visual loss in patients with diabetic retinopathy

OBJECTIVE To evaluate the causes of visual loss in the patients with diabetic retinopathy (DR) METHODS The follow-up visual acuity was compared in the 160 patients (198 eyes) with DR who had been treated with laser photocoagulation and vitrectomy The relative factors were analyzed with visual loss RESULTS The severe visual loss occurred in 89 eyes (44 95%)of 160 patients (198 eyes) with DR Of these eyes, 56 eyes (62 92%)had persistent visual loss, and the visual acuity of 33 eyes (37 08%)was improved An average level of hemoglobin and cholesterol was higher in the patients with visual loss than without visual loss ( P <0 01 or P <0 001) The visual loss was obviously interrelated with vitreous hemorrhage, macular edema, and macular damages of retinal pigment epithelium ( P <0 01) CONCLUSIONS Visual loss could occur in the process of treatment of DR Occurrence of visual loss is interrelated with some factors, for example, vitreous hemorrhage, macular edema, macular damage of retinal pigment epithelium, high hemoglobin and cholesterol etc
