尿激酶-肝素系列溶栓治疗不稳定型心绞痛的临床疗效

目的 :对比尿激酶 (UK) 肝素系列溶栓与传统方法治疗不稳定性心绞痛 (UA)的疗效 ,探讨尿激酶溶栓治疗UA的合理方案。方法 :采用随机、分组、对照方法将不稳定型心绞痛分为溶栓组和对照组。研究两组间远期及近期心绞痛的发生频度、程度、心电图改变及再住院率和转化为稳定性心绞痛的机率等。结果 :总共入选UA患者 92例 ,溶栓组及对照组各半。结果显示 ,溶栓组溶栓前后HR、BP等生命体征无改变 ,不增加心肌耗氧量 (P均>0 0 5 ) ,但抗心绞痛作用已明显优于对照组 :减少心绞痛发作的次数、缩短发作持续时间、减轻发作程度、降低ST段下移的幅度 (P <0 0 5或P <0 0 1) ,减少心绞痛发作时伴随的室早、心动过速以及再住院率等。结论 :低剂量缓给UK加肝素维持的溶栓方案适用于不稳定型心绞痛的治疗

Clinical effects of urokinase-heparin serial thrombolytic th erapy on patients with unstable angina

Objective：That a comparison of between urokinase (UK)-heparin serial thrombolytic therapy and conventional therapy affected patients with unstable angina (UA) probed into a reasonable UA treatment program by use of UK.Methods：Adopted the randomized placebo-controlled method,the patients with UA were divided into thrombolytic group (TLG) and control group (C G),and recent recurrence rate and degree of severity of UA,ECG change of both groups ,and the rate of re-hospitalization and that of turning into stable angina were studied.Results：Total 92 patients with UA (divided equally into two gr oups) were enrolled.The results indicated there were no differences in HR,BP and other vital signs and no extra cardiac oxygen uptake needed of TLG between before and after thrombolytic treatment (all P＞0.05)，but anti-angina effects of thrombolytic therapy are obviously better than CG in terms of lessening of r ecurrence times,shorting of lasting time,alleviating of suffering and lowered ST segment (P＜0．05～0．01）,and reducting of the rates of accompanying by ventricular premature beats,tachycardias and re-hospitalization.Con clusion: The thrombolytic therapy program of low dosage and slow medicat ion of UK combined with heparin fit for treatment of UA.
