食管神经内分泌癌8例临床病理分析

目的探讨食管神经内分泌癌的临床病理特点及生物学行为,重点在诊断与鉴别诊断。方法采用光镜及免疫组织化学方法对8例食管神经内分泌癌进行观察分析及随访。结果本组病例中,男6例,女2例。年龄48-72岁,平均年龄58.3岁。7例食道外膜淋巴结转移(87.5％),1例贲门淋巴结转移(12.5％),3例肝内转移(37.5％)。其中1例贲门淋巴转移及3侧肝转移患者3月内死亡(50％),3例食道外膜淋巴结转移患者6月内死亡(37.5％),1例粘膜内肿块患者2年内发现肝转移及恶病质。肉眼溃疡性肿块多见,部分可呈蕈伞状肿块。神经内分泌标记如NSE、CgA和Syn阳性表达。结论食管神经内分泌癌是一种比较少.见的的高度恶性肿瘤,有独特的病理形态和免疫组化标记,早期可发生转移,预后凶险,活检易漏诊。

Clinnicopatholgical analysis of esophageal neuroredocrine carcinoma:a re port of eight cases

Objective To study the Climnicopatholgic and immunohistochemical characteristics of esophageal neuroendocrine carcinoma,with esphasis on its pathological and differentical diagnosis.MethodsThe eight cases of esophagicl carcinoma were investigated by light microscopy and immunohistochemical staining Clinical follow-up was collected Results.The clinnico Patholgical analysis of eight patients with esophageal neuroendocrine carcinoma showed that six wee male and two female,with age from forty-eight to senvety-two yeats old( mean fifiy-eight point three years) One patient died of lymphatic motastasis of gastric cardia and three patients died of metastasis of cancinoma on the liver in postoperation three months.Three patients died of lymphatic metastasis of esophagus in postopera-fion six months.One patient had metastatic carom m the liver and showed cachesia after two years Mass is that the most common is a ulcerating lesion and the.second most frequent pattem is afungating polypoid The neuroendocrine markers,such as NSE,CgA and Syn,were expressde in all the tumor cells.Conclousion Neuroendocrine carcinoma in esophagus is are and highly malignant tumor with distinct palhological features and immunohistochemistry marker.It is aggressive with early metastases and a poor prognosis.It was often misdiagnosed bispsy.
