腹部急性肝脾创伤的CT诊断

目的探讨闭合性及开放性肝脾创伤的CT表现及诊断价值。材料与方法回顾性分析了48例经手术和临床证实的肝脾创伤的CT表现。结果撕裂伤肝4例，脾19例；实质内血肿肝5例，脾6例；包膜血肿肝6例，脾9例；断裂或碎裂伤3例；腹腔内积血肝周7例，脾周5例，肝肾隐窝1例，肝脾复合伤4例。结论肝脾创伤CT表现类型相似，CT能明确创伤病变的部位、性质、程度，敏感性和特异性高，为临床治疗提供重要信息，为腹部创伤最重要的检查方法之一。

CT Diagnosis of Acute Hepatic and Splenic Trauma (An Analysis of 48 Cases)

Purpose: To discuss the CT appearance and diagnostic value of closed and open hepatic and splenic trauma. Materials and Methods: We retrospectively analyzed the sign of CT of forty eight cases with abdominal hepatic and splenic trauma. Results: CT scans showed that hepatic laceration was in 4 cases, the splenic laceration was in 19 cases. Hematoma of the hepatic essence was in 5 cases, the splenic essence was in 6 cases. Hematoma under the fibrous capsule of the hepar and spleen were in 6 cases and 9 cases. There were peritoneal blood in 13 cases, 7 cases with hepatic surround, 5 cases with splenic surround, 1 case with hepatonephric recess. Hepatic and splenic complex trauma was in 4 cases. Conclusion: The signs of CT of the hepatic trauma are the same as splenic, the character and degree of traumatic lesions can be definitely diagnosed by CT. CT scans have high sensitivity and specificity and can provide important information in clinical therapy of the disease. So it is useful in making the diagnosis of traumatic abdomen.
