胎肺成熟度测定在妊娠期糖尿病患者分娩前的应用价值

目的:探讨妊娠期糖尿病(GDM)孕妇终止妊娠前胎肺成熟度测定的临床价值。方法:选择63例GDM孕妇为研究对象,根据血糖控制情况分为血糖控制满意组(A组)和不满意组(B组);以同期孕周相匹配的正常妊娠孕妇30例为对照组(C组),于终止妊娠前行羊水泡沫试验,比较3组胎肺成熟度及新生儿呼吸窘迫综合征的发生率。结果:血糖控制满意组孕妇胎儿肺成熟与正常妊娠组无显著差异,血糖控制不满意组孕妇胎儿肺成熟较血糖控制满意组延迟。结论:血糖控制满意的GDM孕妇妊娠37周后胎肺已成熟,终止妊娠前不需行羊膜腔穿刺做胎肺成熟度测定;对血糖控制不满意的GDM孕妇在择期终止妊娠前建议常规行羊膜腔穿刺术测定胎肺成熟度,并同时给予羊膜腔注射促胎肺成熟治疗。

The clinic value of the prepartal fetal pulmonary maturity test to the pregnant woman with gestational diabetes mellitus

Objective:To evaluate the clinic value of the prepartal fetal pulmonary maturity(FPM) test to the pregnant woman with gestational diabetes mellitus(GDM).Methods:63 pregnant women with GDM were enrolled and divided into Group A with well-controlled glucose and Group B without well-controlled glucose.And 30 normal pregnant women with the same pregnant weeks were enrolled in Group C,as the control group.The foam tests of amniotic fluid were performed before the termination of pregnancy to the women in these 3 groups to compare the FPM and the rate of neonatal respiratory distress syndrome.Results:There was no difference in the FPM between Group A and Group C.Compared to Group A,the FPM was delayed in Group B.Conclusion:The fetal pulmonary of the women in Group A is mature in the 37th pregnant week,there is no need to do the amniocentesis for the test of FPM.The amniocentesis for the test of FPM and the intra-amniotic injection to promote the maturation of the fetal pulmonary are suggested for the pregnant women in Group B before the termination of pregnancy.
