彩色多普勒超声对新生儿完全性大动脉转位的诊断价值

摘    要:目的 了解新生儿完全性大动脉转位(TGA)的超声特点。评价超声在TGA早期诊断中的价值。方法 回顾分析35例新生儿TGA的超声表现并与手术、心导管和尸解结果对照。结果 单纯TGA6例,合并室间隔缺损、房间隔缺损、动脉导管未闭(PDA)20例,房室间隔缺损1例,肺动脉狭窄1例,肺干静脉畸形引流1例,三尖瓣闭锁2例,右位心2例,冠状动脉异常1例,合并3种以上其他先心病共13例。大动脉位置关系与4例手术、1例心导管、2例尸解结果一致。1例PDA彩超漏诊。结论 新生儿TGA需早期诊断,超声检查无创、准确为临床提供可靠信息。

Abstract:Objective To evaluate echocardiography in early diagnosis by studying echocardiography features of TGA in newborns. Methods 35 cases of neonatal TGA were analyzed retrospectively and compared with the findings of operation, catheterization and autopsy. Results Among the 35 cases,20 cases were with ventricular septal defect(VSD),atrial septal defect(ASD) and patent ductus arteriosus(PDA).1 case with endocardial cushion defect(ECD) ,1 case with pulmonary valve stenosis(PS),1 case with total anomalous pulmonary venous connection(TAPVC),2 cases with tricuspid atresia(TA) and 1 case with anomalous origin of coronary artery.13 cases were with more than three kinds of other congenital heart diseases. Echocardiography showed the changes of the great arteries were compatible with those seen under 4 cases of operation,1 case of catheterization and 2 cases of autopsy.1 case of PDA was missed by Echocardiography. Conclusion TGA in newborns should be early diagnosed. Echocardiography is an invasive and reliable technique and it can provide accurate informations for diagnosis of newborns with TGA.
