特异性IgG4用于食物过敏原的检测

摘    要:目的探讨特异性IgG4检测在食物过敏原筛查中的应用。方法用ELISA法分别检测51例慢性湿疹患者血清中10组食物过敏原特异性IgG4、特异性IgE。结果51名患者中,食物过敏原特异性IgG4阳性35例(68.6%),食物过敏原特异性IgE阳性45例(88.2%),两者差异显著(P<0.05)。10组过敏原中,虾、鱼类过敏原特异性IgE检出率显著高于特异性IgG4(P<0.05)。蛋、牛奶、大豆、猪牛羊肉特异性IgG4检出率明显高于特异性IgE(P<0.05)。西红柿、杏仁、花生、菠萝特异性IgE和特异性IgG4检出率无差异。结论特异性IgG4不能取代特异性IgE检测食物过敏原,但可作为一补充检查,为临床提供更多、更真实的诊疗依据。

Abstract:Objective To investigate the clinical application of examination of specific IgG4 to food allergens.Methods Specific IgG4 as well as specific IgE to ten kinds of food allergens in sera of 51 patients with chronic eczema was examined by ELISA.Results Food allergen specific IgG4 was positive in 35 patients (68.6%) and food allergen specific IgE was positive in 45 patients(88.2%)of the 51 cases (P<0.05).Among the ten food allergens the positive rates of allergen specific IgE to shrimp and fish were remarkably higher than those of allergen specific IgG4 (P<0.05).The detection rates of allergen specific IgG4 to eggs,milk,soya,pork,lamb and beef were significantly higher than those of allergen-specific IgE (P<0.05).There was no difference between specific IgE and specific IgG4 to tomato,almond,peanut and pineapple.Conclusion The examination of specific IgG4 to food allergens could not replace the examination of specific IgE,but it may be a supplementary examination to provide more information for the diagnosis and therapy of chronic eczema patients.
