贲门癌手术径路的合理选择

目的 研究贲门癌选择各种手术径路的临床价值。方法 对本组贲门癌患者，术前进行充分的讨论，依据病灶范围、全身情况，选择适合的手术径路施术，对手术彻底性、切除率、切缘阳性率、术后总并发症、吻合口瘘、心肺功能影响等进行分析。结果 本组手术彻底性高，手术切除率为９５％，全胃切除率１２％，淋巴结转移率８４．６％，总并发症８．７％，吻合口瘘０．４％，心血管并发症３％，呼吸系统并发症４．４％，住院死亡率０．４％

RESEARCHON DIFFERENT ROUTES OF SURGICAL OPERATION OF CARCINOMA OF GASTRIC CARDIA

Objective To study the different routes of surgical operation of carcinoma of gastic cardia and its clinical value. Methods Careful preoperative discussion was necessary. The selection of the routes and types of operations was based on the different scopes and extents of the cancer leision and the general condition of patients. These were carefully analyzed in all 765 cases.Results The thoroughness of the operation was high. The rate of resectabilty was 95 %. Total gastrectomy was done in 12 % of cases. Lymph node metastasis was 84.6 %. Postoperative complication was 8.7%:leaking of anastomosis 0.4%, heart and vascular complication 3 % and respiratory system complication 4.4%. Cancer cells were found along the margin of resection in 0.69%, 0.4% of patients died during hospitalization. Conclusion In ordoer to get a high rate of resectablity and low postoperative mortarity and morbidity, a best route and type of operation must be carefully selected based on a thorough preoperative examination of patients and a detailed preoperative discussion.$$$$
