角膜缘干细胞移植联合丝裂霉素C治疗翼状胬肉临床分析

摘    要:目的:探讨角膜缘干细胞移植术中联合丝裂霉素C治疗翼状胬肉的治疗效果。方法:对474例(492眼)采用3种不同术式分3组行手术治疗。Ⅰ组:单纯翼状胬肉切除术;Ⅱ组:翼状胬肉切除术+自体角膜缘干细胞移植术;Ⅲ组:翼状胬肉切除术+自体角膜缘干细胞移植术+丝裂霉素C。经过6个月~5年的对比观察。结果:3种术式比较:单纯翼状胬肉切除术92眼,治愈62眼,复发30眼,复发率32.6%;翼状胬肉切除术+自体角膜缘干细胞移植术119眼,治愈104眼,复发15眼,复发率12.6%;翼状胬肉切除术+自体角膜缘干细胞移植术+丝裂霉素C手术281眼,治愈276眼,复发5眼,复发率1.8%。结论:翼状胬肉切除术+自体角膜缘干细胞移植术+丝裂霉素C治疗翼状胬肉复发率明显低于其他术式,且术后角、结膜上皮修复快,视力恢复好。

Clinical Analysis of Limbal Autograft Transplantation with Mitomycin C Therapy of Pterygium
Abstract:Objective:Analyze the clinical efficacy of surgical method to treat pterygium by pterygium excision with limbal autograft transplantation with mitomycin C in the operation. Methods:474 (492 eyes)pterygium patients admitted in our hospital, according to different surgical methods were divided into three groups: Ⅰ group: simple excision of pterygium, Ⅱ group: pterygium excision combined with limbal autograft transplantation, Ⅲ: pterygium excision with limbal autograft transplantation with mitomycin C in the operation, Followed up for 6 months to 36 months.Record postoperative recurrence of pterygium, corneal epithelial healing time and postoperative visual acuity, Statistical analysis of differences results of each surgical method.Results: The comparison of the recurrence rate of different pterygium group: Ⅰ group(92 eyes) had 30 eyes recurrence, the recurrence rate was 32.6%; Ⅱ group(119eyes) had 15 eyes recurrence ,the recurrence rate was 12.6%; Ⅲ group(281 eyes) had 5 eye recurrence, the recurrence rate of 1.8%. After statistical analysis, the recurrence rate among the three groups has significant difference. Conclusions: Pterygium excision with limbal autograft transplantation with mitomycin C in the operation is a better treatment to lower the recurrence rate of pterygium and better improvement visual acuity and corneal epithelial repair.
