舒芬太尼预处理对大鼠心肌缺血再灌注损伤的影响

摘    要:目的探索舒芬太尼预处理对大鼠心肌缺血再灌注损伤的作用。方法健康雄性SD大白鼠50只,随机分为5组。采用结扎左冠状动脉前降支30分钟再灌注90分钟的方法制备心肌缺血再灌注模型。I/R组为缺血再灌注对比组,IPC组为缺血预处理组,SPC组为舒芬太尼预处理组,将其分为三个亚组,低剂量组(LS组0.20μg/kg);中剂量组(MS组2.0μg/kg);高剂量组(HS组5.0μg/kg)。应用Even’s blue-TTC法检测心肌梗死范围,以梗死区心肌重量/缺血区心肌重量(IS/AAR%)来表示。于实验结束时(再灌注末),利用硫代巴比妥酸反应法(TBA)测定MDA浓度,利用黄嘌呤氧化酶法测定SOD活性。结果与1/R组比较,IPC组与SPC三个亚组的心肌梗死面积(IS/AAR)均有降低,其中舒芬太尼预处理组(SPC)三个亚组相互比较中,高剂量(HS)组IS/AAR的降低最为明显。与1/R组比较,IPC组及SPC三个亚组的MDA浓度均有下降,SOD活性均有升高。结论舒芬太尼预处理可减轻大鼠心肌缺血再灌注损伤,减少心肌梗死面积,抑制氧化产物的增加,提高机体的抗氧化能力,且呈剂量依赖性。

Abstract:Objective To investigate the effects of Sufentanil preconditioning on myocardial ischemia reperfusion injury in rats in vivo.Methods To randomly divide 50 male SD rats equally into 5 groups,including ischemia-reperfusion group(I/R group),ischemic preconditioning group(IPC group),high dose Sufentanil preconditioning group(HS group,5.0 μg/kg),medium dose Sufentanil preconditioning group(MS group,2.0 μg/kg) and low dose Sufentanil preconditioning group(LS group,0.20 μg/kg).The left anterior descending coronary artery(LAD) of rats in five groups are ligated for 30 minutes and are re-perfused for 90 minutes.To measure the myocardial infraction size(IS/AAR%) with double-staining through Evan's blue and Triphenyltetrazolium chloride,and to calculate the concentration of the serum malondialdehyde through Thiobarbituric acid(TBA) reaction method and the activity of the superoxide dismutase(SOD) through xanthine oxidase reaction method.Results Comparing with Group 1/R,the myocardial infraction size(IS/AAR%) in Group IPC,HS,MS and LS all reduced in different levels.Among the Group HS,MS and LS,the infraction size in Group HS reduced most significantly.Comparing with Group 1/R,the concentration of the serum malondialdehyde in the other four groups all reduced and the activity of the superoxide dismutase increased.Conclusion Sufentanil preconditioning can reduce myocardial infarct size,decrease the concentration of the serum malondialdehyde and increase the activity of the superoxide dismutase.Therefore,Sufentanil preconditioning has protective effects on Myocardial Ischemia-Reperfusion Injury in rats in vivo,and the effects are dose dependent.
