低浓度甲磺酸罗哌卡因联合芬太尼在分娩镇痛的应用

目的探讨低浓度甲磺酸罗哌卡因联合芬太尼经自控镇痛泵技术进行分娩镇痛的有效性与安全性。方法选在我院2009年1月至2010年1月间主动要求行分娩镇痛、健康、单胎、足月临产初产妇50例为镇痛组,随机选择同期条件相当未实施分娩镇痛的孕妇50例作为对照组。镇痛组分娩前给予低浓度甲磺酸罗哌卡因加芬太尼,对照组不行无痛分娩,宫口开全后停止注药,采用视觉模拟疼痛评分（VAS）,记录两组疼痛程度、产程时间、分娩方式、新生儿Apgar评分情况。结果镇痛组镇痛后VAS评分明显下降、第一产程明显缩短（P〈0.05）,而第二产程、第三产程、分娩方式及新生儿Apgar评分与对照组相比无显著性差异（P〉0.05）。结论低浓度甲磺酸罗哌卡因联合芬太尼经自控镇痛泵技术进行分娩镇痛,可获得满意的镇痛效果,是一种安全有效的方法,值得临床推广应用。

Application of of low concentration ropivacaine mesylate combined with fentanyl in labour analgesia

Objective To observe the effect of low concentration ropivacaine mesylate combined with fentanyl for patient-controlled epidural analgesia on labour analgesia. Methods From January in 2009 to January in 2010,50 patients of epidural analgesia in labor,health,single fetal and the term of the provisional maternal were enrolled into trial group. 50 patients without drug analgesia after delivery were chosen into control group with only general treatment. Patients in trial group received low concentration ropivacaine mesylate combined with fentanyl with controlled epidural analgesia. Then visual analogue scales （VAS） , the time of labor course, the pain of childbirth, mode of delivery and other indicators were observed and compared between two groups. Results The VAS in trial group was significantly lower than that in control group（P 〈 0. 05）. There were significant differences in the first stage of labor,and no differences in the second and third stage of labor,mode of delivery and Apgar between two groups （ P 〉 0. 05 ）. Conclusion The effect of combined ropivacaine mesylate and fentanyl used for epidural analgesia in labour is satisfied, and it is worthy in clinical application.
