抑郁对COPD患者运动能力影响的研究

目的探讨抑郁对COPD患者运动能力影响的研究。方法选取COPD患者91例，明确抑郁或者非抑郁患者80例，根据汉密尔顿抑郁量表评分分为对照组和抑郁组，进行6分钟步行试验，在实验开始5min和实验结束2min后检测患者的心率、血压、呼吸频率和Borg指数，计算患者在规定的时间内所步行的距离。结果对照组患者和抑郁组患者实验前的各项指标如差异均如性别比例、年龄分布、BMI、FEV1、FVC、FEV1／FVC两组间的差异均无统计学意义（t值分别为0．050、1．416、1．721、0．041、0．089、0．028，P值均〉0．05）；对照组和抑郁组患者在实验后心率、收缩压、呼吸频率和Borg指数的差异具有统计学意义（t值分别为6．814、18．531、7．694、2．848，P值均〈0．05）；对照组患者组内实验前和实验后的心率、收缩压和Borg指数的差异具有统计学意义（t值分别为16．715、4．169、3．138，P值均〈20．05）；抑郁组患者组内实验前和实验后的心率、收缩压、呼吸频率和Borg指数的差异具有统计学意义（t值分别为21．163、8．372、5．619、19．174，P值均〈0．05）；步行距离两组间的差异具有统计学意义（t=24．851，P〈0．05）。结论抑郁会对COPD患者运动能力造成明显的影响，在临床治疗上，心理治疗需要引起重视。

Effect of depression on exercise capability in patients with chronic obstructive pulmonary disease

Objective To investigate the effect of depression on exercise capability in patients with chronic obstructive pulmonary disease （COPD）. Methods In 9I cases of COPD patients,clear depression or non depressed patients with 80 cases, 6MWT was performed and were divided into the control group and the depression group according to HAMD score, heart rate, blood pressure, respiratory rate and Borg index were calculated at 5min before the beginning of the experiment and the end of the experiment after 2 rain,and the walk distance within a prescribed time were calculated. Results There was no statistical difference of sex ratio, age distribution, BMI, FEV1, FVC, FEV1/FVC in the control group and the depression group before test （t = 0. 050,1. 416,1. 721, 0. 041, 0. 089, 0. 023 , all P 〉0.05）; there were statistical difference in index of the control group and the depression group after test （ t =6. 814,18. 531, 7. 694,2. 548, all P 〈0.05） ; there were significant differences in the heart rate, systolic pressure and Borg index of control group patients before and after the test （t =16.715,4.169,3.138,allP〈0.05）; there were significant differences in the heart rate, systolic pressure, respiratory rate and Borg index of depression group patients before and after the test （ t = 21.163,8.372,5.619,19.174, all P 〈 0.05 ） ; there were statistical difference in walking distance between the two groups （ t = 24. 851, P 〈0.05）. Conclusions Depression can cause a significant impact on COPD patients exercise ability,in the clinical treatment,psychological treatment should pay attention to.
