老年人消化性溃疡并发出血的危险因素分析

目的分析影响老年消化性溃疡并出血的主要危险因素。方法根据患者是否出血，将370例患者分为出血组和未出血组，回顾两组的·临床资料，统计分析年龄、性别、吸烟史、饮酒史、基础疾病情况、溃疡部位、溃疡直径、溃疡数量、服用NSAIDs情况、幽门螺杆菌监测情况，以总结高危因素。结果两组患者在基础疾病、是否服用NSAIDs、腹痛、溃疡部位方面有统计学差异，P〈0．05。多因素Logistic回归分析表明：性别为女性、基础疾病、服用NSAIDs、复合性溃疡和无腹痛是导致老年消化性溃疡并出血的高危因素。结论女性患者、合并有基础疾病、无腹痛、服用NSAIDs和复合性溃疡是导致老年人消化性溃疡并发出血的主要因素。在临床诊断和治疗中，要根据这些高危因素，提早介入诊断和治疗，以提升治疗效果。

Analysis on the Risk Factors of Elderly Peptic Ulcer Complicated with Bleeding

Objective To analyze the major risk factors of peptic ulcer complicated with bleeding in elderly. Methods 370 elderly patients with peptic ulcer were divided into bleeding group and non-bleeding group. The clinical data were analyzed retrospectively to summarize the age, gender, smoking history, drinking history, underlying disease condition, ulcer, ulcer diameter, ulcer number of cases taking NSAIDs, Helicobocter pylori monitor, so as to analyze the high risk factors. Results Underlying disease, whether taking NSAIDs, abdominal pain were the risk factors of peptic ulcer complicated with bleeding （P 〈0.05）. The bleeding risk factors included female sex, underlying diseases, taking NSAIDs, complex ulcer and non-ulcer pain （P〈0.05）. Conclusions Female patients, associated with underlying diseases, no abdominal pain, taking NSAIDs and older complex ulcer peptic ulcer bleeding are the major factors to cause peptic ulcer complicated with bleeding. In the clinical diagnosis and treatment, early diagnosis and treatment interventions according to these risk factors should be taken to enhance the therapeutic effect.
