应用咽声反射测量咽壁顺应性的新方法

摘    要:目的探讨咽声反射仪测量咽壁顺应性的新方法。方法18例经多导睡眠监测(polysomnography,PSG)诊断明确的阻塞性睡眠呼吸暂停低通气综合征(obstructivesleepapneahypopneasyndrome,OSAHS)患者作为观察组,36例正常健康作为对照组。利用咽声反射仪分别记录呼气相及吸气相的咽声反射曲线,取4.8~15cm范围内的平均横截面积及容积,计算顺应性.比较两组测试者咽壁顺应性的差异。结果观察组上气道顺应性平均值为0.42,对照组为0.27,差异有统计学意义(t=4.77,P〈0.001)。结论利用咽声反射测量呼吸相及吸气相上气道设定区段的平均横截面积及容积来计算咽壁顺应性是一种可行方法.可为临床提供客观定量指标。

Abstract:Objective To discuss a new method of measuring pharynx wall collapsibility with acoustic pharyngealmetry on patients. Methods Eighteen patients with obstructive sleep apnea and hypopnea syndrome (OSAHS) diagnosed by polysomnography (PSG) were selected as observation group, 36 healthy individuals as control group. At first, acoustic pharynx reflex curve was recorded with acoustic pharyngealmetry, and then mean cross section area and volume 4.8 to 15 cm around were selected and calculated. The difference of pharyrLx wall collapsibility in two groups was compared. Results The mean value of upper respiratory tract collapsibility was 0.42 in the observation group and 0.27 in the control group, with statistically significant difference of collapsibility in the two groups (t = 4.77, P 〈0.001). Conclusions Calculating pharynx wall collapsibility by measuring mean cross section area and volume in given location in pharynx with aoustic pharyngealmetry was a reliable method and could provide objective quantitative index for clinic work.
