| 摘 要： | 目的检测大肠癌患者外周血中细胞角蛋白20（CK20）mRNA的表达，探讨其能否作为判断大肠癌患者外周血微转移及预后的分子生物学指标。方法采用RT—PCR技术检测32例大肠癌患者、11例肠道良性病变患者和20例健康体检者外周血中CK20 mRNA定性和半定量表达。结果（1）CK20 mRNA在大肠癌患者中的阳性率为50．00％，相对系数为（0．89±0．17）；在肠道良性病变患者中阳性率为9．09％，相对系数为0．11；在健康体检者中阳性率及相对系数均为0。大肠癌患者与肠道良性病变患者、健康体检者相比，CK20 mRNA明显增高（P〈0．05）。（2）CK20 mRNA阳性率和相对系数在不同临床分期、远处转移中差异有统计学意义（P〈0．05）；其阳性率在浸润深度、淋巴结转移中无统计学意义，但相对系数有统计学意义（P〈0．05）。阳性者生存率低于阴性者（P〈0．05）。（3）CK20 mRNA与癌胚抗原（CEA）、糖链抗原199（CA199）表达呈正相关（P〈0．05）。结论CK20 mRNA的表达能预测大肠癌患者外周血微转移，可判断预后。
