大黄■虫汤治疗外科术后非感染性发热(血瘀证)疗效研究

目的观察大黄■虫汤治疗外科手术患者术后非感染性发热(血瘀证)的临床疗效并探讨其作用机制。方法68例患者采用随机数字表法分为治疗组与对照组各34例。对照组给予西医常规治疗,治疗组在对照组基础上加大黄■虫汤口服治疗。通过比较两组患者发热次数、体温降至正常时间、完全退热时间、中医证候积分进行疗效评价。结果治疗组临床总有效率为94.11%,高于对照组的73.53%(P<0.05)。治疗组发热次数、用药后体温降至正常时间、完全退热时间均短于对照组(均P<0.05)。两组治疗后中医证候积分与治疗前比较均降低(均P<0.05),且治疗组中医证候积分低于对照组(P<0.05)。治疗组治疗后发热、手术局部刺痛、肢体麻木中医证候积分均低于对照组(均P<0.05)。但两组皮下瘀斑中医证候积分比较,差别则不大(P>0.05)。结论大黄■虫汤治疗外科术后非感染性发热(血瘀证)患者疗效确切,可降低患者发热次数,使患者体温尽早降至正常,缩短患者住院时间。

Study of Dahuang Zhechong Decoction for Non-infectious Fever with Blood-stasis after Surgery

Objective:To investigate the clinical efficacy of Dahuang Zhechong Decoction for postoperative noninfectious fever of surgical patients with blood stasis.Methods:A total of 68 patients were divided into the treatment group and control group,with 34 cases in each group by random number table method.The control group was given conventional western medicine treatment,and the treatment group were added with oral treatment with Dahuang Zhechong Decoction on the basis of the control group.The curative effect was evaluated by comparing the number of fevers,the time for the body temperature to fall to normal,the time for complete fever reduction,and the scores of TCM syndromes between the two groups.Results:The total clinical effective rate of the treatment group was 94.11%,which was higher than 73.53%of the control group(P<0.05).The number of fevers,the time for body temperature to fall to normal after medication,and the time for complete fever reduction in the treatment group were shorter than those in the control group(all P<0.05).The scores of TCM syndromes in the two groups after treatment were lower than those before treatment(all P<0.05),and the scores of TCM syndromes in the treatment group were lower than those in the control group(P<0.05).After treatment,the scores of TCM syndromes of fever,surgical local tingling,and limb numbness in the treatment group were lower than those in the control group(all P<0.05).However,there was little difference in the scores of TCM syndromes of subcutaneous ecchymosis between the two groups(P>0.05).Conclusion:Dahuang Zhechong Decoction is effective in treating patients with non-infectious fever with blood stasis after surgery.It can reduce the number of fever,reduce the patient′s body temperature to normal as soon as possible,and shorten the patient′s hospital stay.
