Correlation between plasma D-dimer and clinicopathological features and prognosis in patients with pancreatic cancer

Objective To analyze the correlation between plasma D-dimer level and clinicopathological characteristics and prognosis in patients with pancreatic cancer. Methods The clinicopathological data of 60 patients with pancreatic cancer admitted to the Second Affiliated Hospital of Jiaxing University between Jan. 2013 and Dec. 2020 was retrospectively analyzed. The gender, age, plasma D-dimer level at initial diagnosis, serum CEA and CA199 levels, TNM stage, tumor differentiation degree and vascular tumor thrombus were collected. Another 30 cases with health examination in our hospital were taken as control. Plasma D-dimer levels were compared between patients with pancreatic cancer and healthy subjects; the correlation between D-dimer level and clinicopathological features of patients with pancreatic cancer was analyzed. Patients with pancreatic cancer were divided into normal D-dimer group (n=18) and elevated D-dimer group (n=42) with plasma D-dimer level of 550 μg/L as the critical value, and the survival difference between the two groups was compared. Results (1) The average plasma D-dimer level in pancreatic cancer patients was significantly higher than that in healthy subjects [(1 574.35±1 482.13)μg/L vs (248.90±183.28)μg/L, t=4.866, P<0.001]; (2) Plasma D-dimer level was closely correlated with TNM stage, tumor differentiation degree, vascular tumor thrombus and serum CEA level (Z=-3.742, -3.994, -3.707, -1.991, P﹤0.05); (3) The median survival in patients with elevated plasma D-dimer level was significantly shorter than that in the corresponding normal group (χ2=4.464, P<0.05). Conclusion For pancreatic cancer, plasma D-dimer levels is closely related to the TNM stage, tumor differentiation, vascular tumor thrombus and serum CEA level, and higher D-dimer level is associated with worse prognosis.
