地塞米松鞘内注射在脑神经微血管减压术后的应用

目的 探讨脑神经微血管减压术后头痛、发热的原因及腰椎穿刺联合地塞米松鞘内注射在处理头痛、发热中的应用价值.方法 收集脑神经微血管减压术后头痛、发热患者160例,按照随机数字表分为单纯腰椎穿刺组(对照组)72例,腰椎穿刺联合地塞米松鞘内注射组(治疗组)88例,比较两组患者治疗后头痛、发热缓解情况.结果 腰椎穿刺治疗后24 h,对照组头痛分级0级12人、1级24人、2级26人、3级10人,治疗组依次为74人、10人、4人、0人;对照组发热分级正常32人、低热26人、中度发热12人、高热2人,治疗组依次为80人、8人、0人、0人,两组比较差异均有统计学意义(Z头痛=-8.622,P＜0.001;Z发热=-6.515,P＜0.001).在腰椎穿刺次数上,对照组平均次数为2.8次,治疗组为1.5次.结论 脑神经微血管减压术后头痛、发热常为无菌性脑膜炎反应所致,腰椎穿刺联合地塞米松鞘内注射对头痛、发热具有显著的治疗效果.
