| 摘 要： | 目的 探索首诊医师负责制在慢性病患者健康管理中的效果,为其他地区的慢性病健康管理提供参考.方法 将疾病诊疗中的"首诊医师负责制"原理引入到慢性病患者健康管理中的各个环节.通过现场问卷调查和日常工作记录收集方法,了解慢性病患者健康管理首诊医师负责制的实施效果.结果 首诊医师负责制实施后,首诊测血压比例达到94.52%,筛查出的慢性病患者人数同比实施前增长228.76%,纳入管理人数同比增长169.23%,管理效果也明显改善.结论 首诊医师负责制是一种有效的慢性病患者健康管理方式,能够促进社区慢性病健康管理工作的落实.
