手术患者术前饮酒与健康状况的流行病学调查

目的 调查外科手术患者术前饮酒状况与其健康的关系,为饮酒的手术患者实施合理的术前干预提供依据.方法 采用统一调查问卷,根据世界卫生组织对饮酒的定义将手术患者分为饮酒组和对照组,以面对面询问的形式调查840例手术患者过去饮酒及其健康状况.对可能与饮酒有关的健康问题进行单因素和多因素Logistic回归分析,以寻找两者的关系.结果 被调查的840例手术患者中,399例饮酒(47.5%),其中男240例,女159例,男、女饮酒率分别为67.2%(240/357)和32.9%(159/483).单因素分析表明心脏病(OR值=0.501)、高血压病(OR值=0.421)、脂肪肝(OR值=3.192)、高脂血症(OR值=0.481)、肥胖(OR值=2.138)是手术患者术前伴随的健康问题.多因素Logistic回归分析表明与饮酒有关的健康问题有心脏病(OR值=0.49)、糖尿病(OR值=0.07)、脂肪肝(OR值=4.31)、高脂血症(OR值=0.42).结论 入院外科手术患者饮酒已经成为不可忽视的重要问题,且严重影响了心血管系统的功能,手术与麻醉的实施增加了相应的风险,须引起足够的重视,做好麻醉与术前的评估和准备.
