Abstract: | The effect of left ventricular function on changes in plasmavolume during acetate and bicarbonate dialysis was studied instable, chronic dialysis patients. Preservation of plasma volumein patients with a normal left ventricular function (mean circumferentialfibre shortening velocity (VcF) 1 circ/s) was significantlyless during the first hour of acetate dialysis than during bicarbonatedialysis. However, in patients with impaired left ventricularfunction (VcF < 1 circ/s) the decrease in plasma volume wasmore pronounced during acetate when compared to bicarbonatedialysis. This resulted in a decreased ultrafiltration volumeand haemodynamic instability in these patients during acetatedialysis. The fibre shortening velocity increased during acetateand bicarbonate dialysis in patients with a normal left ventricularfunction, whereas in patients with impaired left ventricularfunction fibre shortening velocity increased only during bicarbonatedialysis. In conclusion, in patients with an impaired left ventricularfunction, bicarbonate is preferable to acetate in chronic dialysis. |
