CD_(44)v6、MMP-2在子宫内膜癌的表达及临床意义

目的探讨子宫内膜癌组织中基质金属蛋白酶-2(matrix metalloproteinase,MMP-2)及粘附分子CD44拼构变异体6(CD44splice variant 6,CD44v6)的表达,分析其与子宫内膜癌浸润、转移的关系。方法选取73例子宫内膜癌组织标本及同期正常增生期子宫内膜27例为对照组。用免疫组织化学方法检测CD44v6、MMP-2的表达。结果 CD44v6在EC中的阳性表达显著高于正常增生期子宫内膜(P<0.05);CD44v6与肌层浸润深度有关(P<0.05);CD44v6的阳性表达率随淋巴结转移和病理分级增高有下降的趋势,但差异无统计学意义(P>0.05)。MMP-2在子宫内膜癌中的阳性表达显著高于正常增生期子宫内膜(P<0.05);MMP-2仅与肌层有无浸润有关(P<0.05)。CD44v6和MMP-2在EC组织中的表达呈正相关性(rs=0.268,P<0.05)。结论 EC中CD44v6及MMP-2高表达的患者更易发生浸润、转移,且二者有正协同作用。联合检测CD44v6、MMP-2可作为判断子宫内膜癌恶性程度,预测转移和评估预后的生物学指标。

Expression and clinical significance of CD44v6 and MMP-2 in endometrial carcinoma

Objective To investigate the expression of CD44 splice variant 6(CD44v6) and matrix metalloproteinase-2(MMP-2) in endometrial carcinomas and their significance in the invasion and metastasis of the carcinomas.Methods Expression of CD44v6 and MMP-2 in 73 case of human endometrial carcinoma and 27 normal hyperplasia endometrium tissue were measured by immunohistochemical method.Results The positive rate of CD44v6 expression in endometrial carcinoma was higher than that in normal hyperplasia endometrium tissue that the difference was significant(P<0.05).The higher expression of CD44v6 in carcinoma tissues was associated with greater extent of myometrial invasion(P<0.05).The expression of CD44v6 in endometrial carcinoma showed decreasing trend associated with increasing pathological grading and lymph nodes metastasis but showed no statistic value(P>0.05).The positive rate of MMP-2 expression in endometrial carcinoma was higher than that in normal hyperplasia endometrium tissue that the difference was significant(P<0.05).The high MMP-2 expressions only related to myometrial invasion that the difference was significant(P<0.05).The expressions of CD44v6 was positively correlated to that of MMP-2 in endometrial carcinoma tissues(rs=0.268,P<0.05).Conclusion The high expressions of CD44v6 and MMP-2 in endometrial carcinoma tissues would suggest their roles in the development,invasion and metastasis of endometrial carcinoma.The combination detection of CD44v6 and MMP-2 may be a biological index in judging the malignancy,predicting the metastasis and evaluating the prognosis of endometrial carcinoma.
