腹腔镜下治疗输卵管妊娠112例分析

目的:探讨腹腔镜治疗输卵管妊娠的价值及术式。方法:回顾分析了112例输卵管妊娠腹腔镜治疗的效果。结果:112例中,除2例输卵管间质妊娠和2例输卵管妊娠保守治疗失败外,均在腹腔镜下手术成功。其中,镜下输卵管局部注药保守治疗10例,输卵管开窗取胚15例,81例行输卵管切除术,行清除术4例。出血量最少为0ml,最多为2500ml。结论:只要病情允许,应首选腹腔镜手术,应根据输卵管的病变情况,患者年龄及有无生育要求,采用输卵管妊娠局部注药术,输卵管开窗术,输卵管切除术和清除积血及流产物等术式。

In the treatment of 112 cases of tubal pregnancy laparoscopy

Objective:To find out the value and procedure method of laparoscope in treating tubal pregnancy. Method: Retrospective analysis of 112 cases wih laparoscopy were done in this hospital for 4 years. Result:Of the 112 cases,108 cases were succeeded in laparoscope surgery,the successful rate was 96. 43% (108/112) ,4 cases were failed,including 2 cases of tubal interstitial pregnancy and 2 cases of conservative treatment of tubal pregnang. Among the 108 successful cases, there were 10 cases of conservative treatment under laparoscope by local injection of medicine MTX to tube,ls cases of embryo extraction by opening tubal window,79 cases of tubectomy,4 cases of drawn of products of pregnancy. The minimum amount of bleeding was 0 ml, and the maximum was 2500 ml during the operation course. Conclusion: Laparoscopy may be the first choice, according to pathological type of fallopian tube, patients'age and the opinions for child bearing, local injection of medicine, tubal window operation tubectomy and hematocele and drawn of products of pregnancy, should be selected.
