检测颈动脉粥样硬化患者血清基质金属蛋白酶的意义

摘    要:目的:观察颈动脉粥样硬化(CAS)患者血清中基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-9(MMP-9)及基质金属蛋白酶抑制剂-1(TIMP-1)含量的变化,说明它们与动脉粥样硬化的关系。方法:通过彩色多普勒超声检查及临床判断为CAS患者77例,用高脂饲料加空气干燥术形成家兔颈动脉粥样硬化模型8只。用酶联免疫吸附试验检测CAS患者和家兔模型血清MMP-1、MMP-9及TIMP-1含量,并计数家兔模型血液循环内皮细胞(CEC)水平。结果:CAS患者和家兔模型血清MMP-1、MMP-9水平明显高于正常对照(P〈0.05),而TIMP-1水平则低于正常对照(P〈0.05)。家兔模型造模4周后开始出现CEC明显增多。血清MMP-1与TIMP-1水平呈负相关。结论:CAS患者血清MMP-1、MMP-9及TIMP-1异常代谢可导致血管内皮破坏,与AS形成密切相关。CAS患者检测这三个指标含量有助于从机理上分析CAS的形成原因,同时可用于观察患者的病变程度和疗效评判。

Abstract:Objective: To observe the level of matrix metalloproteinase-1(MMP-1),matrix metalloproteinase-9(MMP-9)and matrix metallo-proteinase inhibitor-1(TIMP-1) in carotid atherosclerosis(CAS) patients,to explain the relation between them and carotid atherosclerosis(CAS).Methods: Seventy-seven CAS patients were diagnosed by color doppler ultrasound,and eight experimental carotid atherosclerotic rabbit models were induced by high cholesterol diet plus air drying.The level of serum MMP-1,MMP-9,TIMP-1 in CAS patients and experimental carotid atherosclerotic rabbit models were assayed by enzyme linked immunosorbent assay.And the level of blood circulate endothelial cells in rabbit models was assayed. Results: The level of serum MMP-1,MMP-9 in CAS patients and experimental carotid atherosclerotic rabbit models was significantly higher than in the control group(P<0.05).The level of serum TIMP-1 in CAS patients and experimental crotid atherosclerotic rabbit models was lower than in the control group(P<0.05).The level of blood circulate endothelial cells in rabbit models increased obviously after four weeks.The level of serum MMP-1 was negative correlated with serum TIMP-1. Conclusion: The abnormal metabolism of serum MMP-1,MMP-9 and TIMP-1 could destroy blood vessel endothelium.There was a relation between MMP-1,MMP-9,TIMP-1 and CAS.To assay three important indexes in carotid atherosclerosis cases could help us analyze the reason of CAS,observe the degree of pathological changes and judge curative effect.
