手足口病重症预警方法临床研究

目的:在手足口病救治过程中探讨一种重症预警方法,能实用地、系统地早期识别手足口病重症患儿,并进行早期干预,敦促治疗关口前移,阻断病情进展,提高愈后。方法:采用自制的"告患儿家长书"、"患儿住院期间病情评估表"、"重症预警指标"等表格按照一定的程序和方法进行病情观察和护理。将收治入院诊断符合非重症手足口病诊断标准的850例患儿作为试验组,在传统护理方法的基础上采用重症预警方法进行护理;将同期同类型未实行重症预警方法的900例患儿作为对照组。比较两组患儿重症发生率、并发症发生率、平均住院天数和平均住院费用的差别。结果:试验组和对照组重症发生率(P<0.005)、并发症发生率(P<0.05)、平均住院天数(P<0.001)、平均住院费用(P<0.001)方面比较,差异均有统计学意义。结论:重症预警方法能早期识别手足口病重症患儿,经早期干预能有效减少手足口病重症发生率、减少并发症、改善愈后,并能缩短患儿住院天数,降低住院费用,有效节约医疗资源,减轻患儿家长负担。

A prewarning method for severe hand-foot-mouth disease

Objective To find a prewarning method in treating and curing hand-foot-mouth disease,by which medical workers can predistinguish severe cases practically and systematically.The key point of treating and curing can be forward leaded by early intervention.The development of severe acute state can be prevented and curative effect can be improved.Method We adopted a set of self-made tables to use in observation,such as Patriarch Information Leaflet,Assessment Form for Inpatient,Prewarning Index for Severe Cases,et al.Patients were randomly divided into two groups,which were The experimental group（n=850）and the control group（n=900）.The patients in the experimental group received prewarning method of severe cases except for general attendance.The patients in the control group only received general attendance.The occurence of severe cases,the complication rates,the average hospital days and the average maintenance fee were compared.Results The occurence of severe cases in the experimental group was lower than that in the control group（P0.005）.The complication rates in the experimental group was lower than that in the control group（P0.05）.The average hospital days and the average maintenance fee in the experimental group were lower than that in the control group（P﹤0.001）.Conclusion This prewarning method could predistinguish severe cases of hand-foot-mouth disease.By early intervention,the occurence of severe cases and the complication rates could be cut down effectively and the curative effect is improved.By reducing the hospital days and maintenance fee,medical resources could be saved and the burden of patriarches could be alleviated.
