CT导向下^1255I粒子组织间植入治疗肺癌

目的 探讨CT导向下放射性^125I粒子组织间植入治疗肺癌的技术方法、临床价值和短期疗效。方法 对本组24例经活检病理组织学证实的无法手术的肺癌患者，先利用治疗计划系统（TPS）制定计划，然后在CT导向下经皮穿刺植入^125I粒子，术后复查CT，详细记录粒子分布情况及并发症。以后每隔1个月复查CT，以观察疗效。结果 短期总有效率95．8％（23／24），其中完全缓解20．8％（5／24），部分缓解75．0％（18／24），无变化4．2％（1／24），病灶进展0（0／24）。出现轻度并发症5例（20．8％），其中少量气胸3例（12．5％），少量出血1例（4．2％），粒子移位1例（4．2％）。复查病例均未发现放射性肺炎或骨髓抑制等严重并发症。结论 CT导向下^125I粒子组织间植入治疗技术安全有效，并发症少，值得推广。

CT guided radioactive iodine - 125 seeds interstitial brachytherapy for lung cancer

Objective To appraise the method,clinical value and efficacy of radioactive iodine-125 seeds interstitial brachytherapy for lung cancer.Methods Twenty-four cases with biopsy proven unresectable lung cancer were treated with computed tomography(CT) guided radioactive ~(125)I interstitial implantation planned by TPS.Post-treatment CT review scans were performed immediately after implantation and the distribution and side effects showed by CT were recorded in details.The effect was observed by CT scans repeated every one monthe after the treatment.Results Total effect rate in a short period was 95.8%(23/24).CR 20.8%(5/24),PR75.0%(18/24),NC 4.2%(1/24) and PD0(0/24).Five cases(20.8%) had side effects,including slight pneumothorax 3 cases(12.5%),slight bleeding 1 case(4.2%) and seed malposition 1 case(4.2%).No serious complications like radiation pneumonia or myelosuppression were detected.Conclusion CT guided radioactive ~(125)I seeds interstitial implantation is a safe,effective method with little side effects and worthy to be spread.
