PTEN、nm23-H1和E-钙黏附蛋白与乳腺癌预后的关系

摘    要:目的 :研究PTEN、nm2 3 H1和E 钙黏附蛋白在乳腺癌组织中的表达并探讨其临床应用价值。方法 :应用S P免疫组化法检测浸润性乳腺癌 (包括导管癌 5 4例 ,小叶癌 2 6例 )组织中上述指标的蛋白表达情况。结果 :正常乳腺组织中PTEN呈高表达 ;远处转移组PTEN高表达率低于无转移组 ,两组间有统计学差异 (P <0 0 5 )。PTEN表达水平与患者年龄、肿瘤最大直径、淋巴结转移、组织学分级及类型无关 ,但在低年龄组和淋巴结转移组PTEN低表达率增高有相关趋势。PTEN低表达可能与乳腺癌预后有关 ,容易复发和远处转移。nm2 3 H1低表达者易发生转移复发 (P <0 0 5 )。E 钙黏附蛋白的表达与淋巴结转移和复发无关 ,与组织学类型有关 (P <0 0 5 ) ,小叶癌表达水平低。结论 :(1)同时检测PTEN、nm2 3 H1的蛋白表达对判断乳腺癌预后具有指导意义。 (2 )E 钙黏附蛋白作为判断预后指标不理想 ,可以作为鉴别小叶癌和导管癌的辅助指标

Abstract:Objective:To study the expression of PTEN, nm23 H1 and epithelial cadherin proteins in the tissues of breast cancer and to investigate their value in clinical application. Methods: The expressions of the above proteins in infiltrating breast cancer tissues were measured by S P immunohistochemical technique including ductal carcinoma ( n =54) and lobular carcinoma ( n =26). Results: The expression of PTEN was high in normal breast tissues. In the distant metastasis group, the high expression rate of PTEN was lower than that of the no metastasis group ( P <0.05). There was statistical difference between the two groups. The expression level of PTEN was not associated with the age, maximum diameter of a tumor, lymph node metastasis, histological grade and types. However, the lower expression rate of PTEN in the lower age group and the lymph node metastasis group had a increasing tendency. The lower expression of nm23 H1 protein may be associated with prognosis of breast cancer. It is easy to recur and metastasize in distance. Patients who have lower expression of nm23 H1 are likely to metastasize or recur ( P <0.05). The expression of E cadherin has no correlation with the lymph node metastasis and recurrence, but it has correlation with the types of histological types ( P <0.05). The expression level of lobular carcinoam is low. Conclusion: The measurement of PTEN and nm23 H1 proteins expression at the same time may have a guiding significance in evaluating the prognosis of breast cancer. E cadherin is not ideal in evaluating the index of the prognosis. It can be used as an accessory index in identifying lobular carcinoma and ductal carcinoma.
