髋臼后壁骨折的诊断和手术治疗

我院于１９９０年１月至今共收治１２例髋臼后壁骨折，经Ｘ线摄片和ＣＴ检查，诊断后壁骨折缺损＞４０％９例，脱位５例，半脱位２例，关节内骨块５例。总结了ＣＴ诊断的优点，认为Ｘ线片与ＣＴ结合可提高诊断的正确性。治疗采用支撑钢板固定骨折，获得理想疗效。随访结果：优１０例，良２例。并讨论了手术方法及其优缺点和理论依据。提出对合并关节内游离骨块和关节脱位手法复位失败者急诊手术可获得较好疗效。

Diagnosis and Operative Treatment of Acetabular Posterior Wall Fracture

Twelve cases of acetabular posterior wall fracture (APWF) were reported. Of them, associated dislocation 5 cases, subluxation 2 cases, intraarticular fragements 5 cases, the posterior fragment of acetabulum more than 40% in 9 cases. This report suggested that CT is more useful than X ray film in detecting joint fragment, subluxation and size of posterior fragment. A newly curved buttressing plates were used to treat all of these cases. Follow up results were satisfactory. This paper discussed the advantage, disadvantage, surgical technique and theory basis of buttrssing plate internal fixation for APWF. Authors think APWF with intraarticular fragments and failure reduction of dislocation should be treated on the day of injury to offer best prognosis.
