“寸口三部脉诊法”肺系疾病临床应用现状分析

目的:分析"寸口三部脉诊法"在肺系疾病临床应用文献中的使用频度,了解"寸口三部脉诊法"的使用情况,掌握中医脉诊方法的进展和现状。方法:应用"万方期刊学位""同方期刊学位""维普期刊""中国知网数据库",以新版《中医内科学》中"肺系系统疾病"诊断名称为标题,以"中医""辨证"为主题词与关键词,交叉检索1984年1月—2015年1月符合标准的全部可检索文献,分别以"寸脉""关脉"或"尺脉"为查找词,查找文献内应用"寸口三部脉诊法"诊断病症与辨证施治情况。结果:共检出符合标准的相关文献2984篇,其中使用"三部脉法"进行辨证施治的文献4篇,占所检文献的0.13%;与未应用文献99.87%比较,差异有统计学意义(P0.01),其中1984年—1999年共有2篇,占此年限间所检文章的0.18%,2000年—2015年共有2篇,占0.11%;以2000年为分界点,前15年与后15年文献比较,其使用频度无统计学差异(P0.05)。结论:近30年中医诊治肺系疾病的文献中,大多数医者将"寸、关、尺"三部视为一部,以不分部的"整体脉法"进行辨证施治,而将寸口分为"寸、关、尺"三部的"寸口分候脉法"则仍是寸口诊法中的重要组成部分,其诊断价值可能是针对"寸口整体脉法"无法准确辨证的某些特殊病例进行辨证施治。

Status Analysis of the Clinical Application of "Cunkou Three-Section Pulse Diagnostic Method" in Pulmonary Disease

Objective:To analyze the application frequentness of " Cunkou three section pulse diagnostic method" in the literatures on pulmonary disease,understand the application condition of " Cunkou three section pulse diagnostic method" and get to know the progress and current situation of TCM pulse diagnosis.Methods:Apply the databases of " Wanfang journals and dissertations"," Tongfang journals and dissertations" and "VIP journals",take the diagnostic name of " pulmonary disease" in Internal Medicine of Traditional Chinese Medicine as the title," Chinese medicine" and " syndrome differentiation and treatment" as the subject terms and key words to search all the literatures accorded to the criteria between January 1984 and January 2015.Take the "Cun pulse"," Guan pulse" or " Chi pulse" as the searching word to search the diseases applied " Cunkou three section pulse diagnostic method" and the condition of syndrome differentiation and treatment.Results:There were 2984 articles accorded with the criteria and 4 articles involving " Cunkou three section pulse diagnostic method",accounting for 0.13％;compared with the 99.87％ non-applied articles,the difference had statistical significance (P ＜0.01).There were 2 articles between 1984 and 1999,accounting for 0.18％ of the articles during this period;2 articles between 2000 and 2015,accounting for 0.11％ of the articles during this period.Take the year of 2000 as the demarcation point,the difference of the application frequentness of articles before and after 15 years had no statistical significance (P ＞ 0.05).Conclusion:Among the literatures within 30 years for the treatment of pulmonary disease by Chinese medicine,most doctors take the three sections of "Cun,Guan and Chi" as one section to conduct syndrome differentiation and treatment.However,the " Cunkou separated pulse diagnostic method" was still an important component which divides Cunkou into three sections of "Cun,Guan and Chi".The diagnostic value may lie in the syndrome differentiation and treatment of some special diseases which can not be accurately diagnosed by "Cunkou holistic pulse diagnostic method".
