吻合器吻合术与Dixon手术在直肠癌治疗中的疗效比较

目的比较吻合器吻合术与Dixon手术在直肠癌治疗中的疗效。方法观察30例行Dixon手术患者（Dixon手术组）和37例行吻合器吻合术患者（吻合器吻合术组）手术时间,手术过程中的输血量以及吻合口瘘、吻合口狭窄的发生情况,随访2个月后观察患者日排便次数以及控便能力。结果吻合器吻合术组手术时间小于Dixon手术组,差异有统计学意义（P〈0.05）;两组输血量差异无统计学意义（P〉0.05）;吻合器吻合术组患者发生吻合口瘘的发生率均低于Dixon手术组（P〈0.05）,二者吻合口狭窄发生率差异无统计学意义（P〉0.05）。2个月后吻合器吻合术组患者日排便次数小于Dixon手术组（P〈0.05）,且控便能力正常比例高于Dixon手术组（P〈0.05）。结论吻合器技术在Dixon手术中的运用能节约手术时间,降低术后并发症的发生,提高肛管自制功能,提高患者生存质量,使Dixon手术的指征得以进一步扩大。

The Comparison of Therapeutic Effects of Anastomosis Stapler and Dixon for Rectal Carcinoma

OBJECTIVE Compare the therapeutic effects of anastomosis stapler and dixon for rectal carcinoma. METHOD Operation time, transfusion volume and the occurrence of anastomotic leakage and anastomotic stenosis were investigated both in dixon group and anastomosis stapler group. Defecation times per day and ability of controlling stool were observed two month follow-up. RESULTS Operation time of anastomosis stapler group were less than dixon group（P〈0.05）; there was no statistical difference between these two group of transfusion volume （P〉0.05）; the ration of anastomotic leakage of anastomosis stapler group was lower than dixon group （P〈 0.05 ） ,while anastomotic stenosis was no statistical difference（P〉0.05 ） . Defecation times per day of anastomosis stapler group were less than dixon group（P〈0.05）, and the ration of normal ability of controlling stool was higher than dixon group（P〈0.05）. CONCLUSION Anastomosis stapler for rectal carcinoma could save operation time, decrease the occurrence of common complications, improve the ability of anus, improve the quality of life of patients,and enlarge the dixon for rectal carcinoma indication.
