卡介菌多糖核酸联合NB-UVB照射对特应性皮炎的疗效及外周血嗜酸性粒细胞的作用

摘    要:目的观察卡介菌多糖核酸联合窄谱中波紫外线(NB-UVB)照射后特应性皮炎(AD)患者的疗效及外周血嗜酸性粒细胞计数的变化情况,为治疗特应性皮炎提供指导依据。方法将入选患者随机分为两组,各39例。治疗组予卡介菌多糖核酸联合窄谱中波紫外线照射治疗,对照组仅予窄谱中波紫外线照射治疗,两组的疗程均为8周。采用SCORAD计分系统对AD临床严重程度进行评估;采用视觉模拟尺度(VAS)评分法对瘙痒程度进行评价,并分别于治疗前后检测两组患者外周血嗜酸性粒细胞水平比较。结果治疗8周后,治疗组和对照组的有效率分别为82.05%和53.85%,差异有统计学意义(P<0.01);治疗组外周血嗜酸性粒细胞水平(221.65±43.51)明显低于对照组(294.35±60.21)和治疗前(376.21±119.34),差异均有统计学意义(P均<0.01);但治疗前两组患者的外周血嗜酸性粒细胞水平(376.21±119.34和353.32±131.33)差异无统计学意义(P>0.05)。结论卡介菌多糖核酸联合窄谱中波紫外线照射可明显提高特应性皮炎患者的疗效,显著降低其外周血嗜酸性粒细胞水平。

Abstract:Objective To observe the efficacy of BCG-PSN combined with narrow-band ultraviolet B irradiation for the treatment of atopic dermatitis and the variations of the peripheral blood eosinophil count in patients, which can give guidance for the treatment of atopic dermatitis. Methods Seventy-eight patients with atopic dermatitis were randomly divided into trial group and control group, and each group had 39 patients. The trial group was treated with BCG-PSN and narrow-band ultraviolet B irradiation, and the control group was treated with narrow-band ultraviolet B irradiation only, the treatment course was 2 weeks for both groups. The severity of stroke patients with atopic dermatitis was evaluated according to SCORAD scoring system; the extent of pruritus was evaluated on VAS, and levels of peripheral blood eosinophil between the two groups were detected and compared before and after. Results After 8 weeks treatment, the rates of efficacy were 82.05% in the trial group and 53.85% in the control group, the level of peripheral blood eosinophil (221.65 ± 43.51) was significantly lower than the control group (294.35 ± 60.21) and before treatment (376.21 ± 119.34), the differences were statistically significant (P <0.01); but the levels of peripheral blood eosinophil (376.21 ± 119.34 and 353.32 ± 131.33) in patients before treatment did not have statistically significant difference (P> 0.05). Conclusion BCG-PSN combined with narrow-band ultraviolet B irradiation can improve efficacy in atopic dermatitis and reduce the peripheral blood eosinophil count significantly.
