瘤周水肿明显的脑膜瘤术后高压氧治疗

目的 探讨高压氧(HBO)在瘤周水肿明显的脑膜瘤术后的治疗作用.方法 将232例瘤周水肿明显的脑膜瘤根据术前、术后平衡的原则分为HBO治疗组和对照组,各116例.HBO治疗组在对照组治疗的基础上,早期应用高压氧治疗,术后半个月统计两组患者的机能状态(KPS评分)、术区水肿体积,术后半年统计两组患者术区异常密度影体积和神经功能障碍例数,并做统计学分析.结果 两组患者术后3 d KPS评分及术区水肿体积差异无统计学意义(P>0.05);术后半个月HBO治疗组KPS评分高于对照组,术区水肿体积明显小于对照组,两组比较差异有统计学意义(P<0.05);术后半年HBO治疗组术区异常密度体积小于对照组(P<0.05),HBO治疗组出现过癫痫、失语或轻瘫等神经功能障碍的例数少于对照组(P<0.01).结论 术后早期应用高压氧治疗瘤周水肿明显的脑膜瘤,可减轻水肿体积,提高KPS评分和降低神经功能障碍的发生率,有较好的临床应用价值.

Treatment on meningeoma with peritumoitral brain edema by hyperbaric oxygenation after tumorectomy

Objective To explore the effect of hyperbaric oxygen(HBO) on treating meningeoma with peritumoral brain edema after tumorectomy. Methods According to the principle of holding the balance of preoperative and postoperative, the clinical data of 232 patients with peripheral edema around meningeoma were divided into experimental group and control group, that each were 116 cases. HBO therapy was early applied in experimental group and other therapy was conformed to control group. The function state (KPS score), operative regional edema volume of these groups were performed inductive statistics at half months after tumoreetomy. The cases of abnormal operative regional density image and neurofunctional disturbance at half years after operation were analyzed. Result The KPS score and operative regional edema volume have no significant difference between these two groups 3 days after operation (P>0. 05); the KPS score of experimental group was higher than control group at half months, and its edema volume was smaller(P<0.05) ; the operative regional edema volume of experimental group was smaller than control at half years (P<0.05), and the examples of experimental group who had generated epilepsy, aphasia, incomplete paralysis were smaller than control group ( P <0.01). Conclusion The meningeoma with significal peripheral edema should be applicated early HBO therapy after tumorectomy, and that often obtain better therapeutic efficacy.
