小儿腹股沟斜疝的微创手术116例报告

目的探讨小儿腹股沟斜疝微创手术的方法,评价其临床效果。方法单侧者经外环口,双侧者经两侧外环口连线中点做1cm小切口,行疝囊高位结扎术,并与我院同期传统手术进行对比性研究。结果微创手术116例,113例于门诊顺利完成手术,手术切口1cm,平均术中出血少于3ml,平均手术时间35分钟。总并发症3.45％,其中3例分别因术中损伤膀胱,寻找疝囊困难延长切口,疝囊撕伤延长切口收入院治愈。随访3月-3年,复发1例,无其他并发症。同期78例传统手术,手术切口3-4cm,平均术中出血10ml,平均手术时间60分钟,住院时间5-7天。总并发症11.54％,其中损伤肠管1例,阴囊血肿4例,切口感染2例,随访3月-3年,复发2例。结论小儿腹股沟斜疝微创手术优于传统手术,安全,创伤小,恢复快,手术效果确切。手术适应症的选择及注意手术技巧是手术成功的关键。

A Report on Micro - injured Operation For 116 Cases of Groin Hernia of Child

Objective To discuss the method of groin hernia of the child,evaluate it's clinical effect.Methods:unilateral hernia through external inguinalring,and bilateral hernia through the middle point of two extermal inguinaling,operations of hernioplasty,compare wit traditional operation of our hospital in the corresponding periods.Results micro-injured operation 116 cases,113 cases were operated outpatient,the average incision length is less than 1cm,the average bleeding capacity is less than 3mi,average operation time;35min,total complication;3.45%.3cases were hospitalizated because of bladder trauma,prolong incision of inconvenience with finding hernia vesicle,prolong hernia vesicle avulsion,all cases were clinically cured.There was only one case recurred in 3months-3year Following-up periods.No complication occur.And 78 cases who received the traditional operation in the corresponding periods,the average incision length is 3-4cm.the average bleeding capacity is 10ml,the average operation time:60min,total complication; 11.54%.1 case was intestinal trauma,4cases were scrotal haematoma,2 cases were infecting of incision,two cases recurred in 3months-3year Following-up periods.Conclusion The micro-injured operation of indirect inguinal hernia is better than traditional operation,and it provide with more factors in following:safe,tiny trauma,rapid recovering and definite effect.The key-point of the operation is skill and the choice of indications.
