摘    要:目的 分析犬多部位伤海水浸泡后血流动力学特点,为海水浸泡多部位伤的早期救治提供理论依据。方法 成年杂种犬30只,制作多部位伤动物模型,随机分为致伤浸泡组(n=10)、致伤非浸泡组(n=10)和非致伤浸泡组(n=10)。观察犬多部位伤后在血流动力学变化规律。结果 犬多部位伤海水浸泡后,平均动脉压(MAP)、中心静脉压(CVP)和心输出量(CO)下降等血流动力学明显紊乱。致伤非浸泡组犬MAP和CO早期一过性升高,随后缓慢下降;浸泡非致伤组血流动力学在浸泡早期无明显变化,浸泡后1.5~2小时MAP、CVP和CO等指标出现轻度下降。结论 海水浸泡导致的高渗性脱水、体温过低等病理生理学改变是引起犬多部位伤后血流动力学紊乱,引起犬早期死亡率的主要因素。

Abstract:Objective To evaluate the effect of seawater immersion on hemodynamics changes after multiple trauma.?Methods The canine multiple traumas model was established.?Thirty health adult dogs were divided into tree groups randomly.?Trauma immersion group(n=10) was immersed into artificial seawater after multiple trauma.?Control group 1 (n=10) was subjected to multiple trauma without immersion.?Control group 2 (n=10)only had seawater immersion without any injury.?Hemodynamic changes were observed at different time intervals after injury or seawater immersion.?Results?Trauma immersion group showed obvious hemodynamic disorder.?MAP,CVP and CO declined obviously and speedily.?MAP and CO of control group 1 raised temporarily at very early stage and declined slowly soon.?The control group 2 had no obvious hemodynamic changes at early stage of immersion.?But MAP,CVP and CO declined slightly after 1.5 to 2 hour seawater immersion.?Conclusion Seawater immersion is the main factor leading to the disturbance of hemodynamic after multiple trauma.
