3/4肾切除并异丙肾皮下注射致慢性心衰合并肾衰模型的制备

目的:研究大鼠3/4肾切除并异丙肾上腺素(ISO)皮下注射致慢性心衰合并肾衰模型的制备.方法:雄性SD大鼠经“两次手术切除法”行3/4肾切除,1周后分成2组,分别皮下注射ISO(70 mg/kg或100 mg/kg,分2次,间隔24 h),对照组给予假手术并注射生理盐水,4周后比较各组大鼠存活率、心衰及肾衰情况.试剂盒法测定血清肌酐、尿素氮,Bradford法测定24 h尿蛋白量,左心室插管术测定左室收缩压(LVSP)、左室舒张压(LVDP)、左室舒张末压(LVEDP)及左室压力最大上升及下降速率(±dp/dtmax).称重法测定心脏重量参数.切片HE染色观测心肌病理情况.结果:对照组大鼠无死亡,大小剂量模型存活率分别为73％、87％,与对照组相比,大小剂量模型组大鼠血清各项指标、尿蛋白明显升高,LVSP下降,LVDP和LVEDP均上升,心脏重量参数比升高,提示左室舒张和收缩功能损伤,发生左室心衰和左室心肌肥厚重构.且大剂量组与小剂量组比较心肾功能差异有统计学意义.结论:3/4肾切除并ISO(100 mg/kg)皮下注射较ISO(70 mg/kg)皮下注射,更易同时诱发大鼠左室心衰及肾衰.

Establishment of Rat Mode of Chronic Heart Failure with Renal Failure Through Isoproterenol Injection and 3/4 Nephrectomy

Objective: To establish the mode of chronic heart failure(CHF) with renal failure in rats through isoproterenol(ISO) injection and 3/4 nephrectomy.Methods: Male SD rats underwent 3/4 nephrectomy,and after a week,rats were randomly divided into two groups according to different treatment as twice subcutaneous injections of 70 mg ISO(low dose group) or 100 mg ISO per 100 gram bodyweight(high dose group).The control group only received sham-operation and injection of the same volume of normal saline.Four weeks later,rat survival rates were compared.The serum creatinine(Cr),urea nitrogen(BUN),and urine protein were determined,and LVSP,LVDP,LVEDP,dp/dtmax,and-dp/dtmax were detected through left ventricular cannulating.The left ventricular weight index(left ventricular weight to body weight) was calculated.Results: Compared with that respectively in control group,Cr,BUN,urine protein,LVDP,LVEDP,and left ventricular weight index in both nephrectomy groups were increased markedly,while LVSP and dp/dtmax were decreased.These indices also had significant difference between low dose group and high dose group.Survival rates of low dose group and high dose group were 87% and 76% respectively.Conclusion: Three quarter nephrectomy with subcutaneous injection of ISO,especially with high dose ISO,can induce rat heart failure and renal failure.
