髋臼双柱骨折内固定生物力学稳定性的有限元分析

摘    要:目的:评估双柱钛板( A)、前路特殊塑形钛板加方形区螺钉( B)及前路特殊塑形钛板加方形区螺钉联合后柱螺钉(C)3种内固定治疗髋臼双柱骨折的生物力学稳定性。方法利用有限元建模软件建立髋臼双柱骨折分别经3种不同方式内固定术后模型,并进行加载分析。结果分别在坐、站位下,在相同加载方式时3种内固定模型髋臼处的最大应力、最大位移及骨折线上节点位移均数比较,均为A>B>C。骨折线上节点位移在坐位时,A与B、C的差异均具有明显统计学意义( P<0.05),但B与C及站位时三者的差异无明显统计学意义( P>0.05)。结论前路特殊塑形钛板加方形区螺钉联合后柱螺钉内固定治疗髋臼双柱骨折具有良好的生物力学稳定性。

Abstract:Objective To evaluate the biomechanical stability of three internal fixations in treatment of double column acetabular fracture ,including double column titanium plate (A),anterior special -shaping titanium plate plus quadrilateral screws ( B) ,and anterior special-shaping titanium plate plus quadrilateral screws and posteri-or column screws ( C).Methods Double column acetabular fracture models were built by using finite element modelling software ,then loading analysis was applied .Results In the acetabulum ,during the sitting and standing position,the maximum force,the maximum displacement and the mean node displacement on the fracture line of these three internal fixations showed A 〉B〉C under the same loading method .However,the node displacement on the fracture line during sitting position showed statistically significant differences between A and B ,C internal fixa-tions(P0.05).There was also no statistical significance in standing position among these three groups (P〉0.05).Conclusion The technique of u-sing anterior special-shaping titanium plate plus quadrilateral screws and posterior column screws has good biome -chanical stabilities for the treatment of double column acetabular fracture .
