不全性药物流产药物保守治疗(附149例病例分析)

目的　探讨药物流产时不全流产经保守治疗的临床效果。方法　 14 9例不全性药物流产患者随机分二组。A组为黄体酮治疗组 :肌注黄体酮和口服安宫黄体酮治疗。B组为氨甲喋呤治疗组 :肌注MTX或口服MTX治疗。观察两组疗效、成功率。结果　二组治疗成功者A组为 79 0 7%( 68/86) ;B组为 96 82 %( 61/63 )。特别是HCG定量较低者有效率较高。A组为 86 89%( 5 3 /61) ;B组为 10 0 %( 3 2 /3 2 )。两组比较差异有显著性 (P <0 0 5 )。结论　药物流产时不全流产病例 ,在阴道出血不多情况下可以予保守治疗。并且可根据HCG水平 ,选择合适的保守治疗方法

A Clinical Study on Conservative Treatment of Incomplete Pharmaceutical Abortion

Objective To study the clinical effects of conservative treatment of incomplete pharmaceutical abortion. Methods 149 cases of incomplete pharmaceutical abortion were randomly divided into two groups. Group A(86 cases) was intramuscularly given progesterone or orally medroxyprogesterone. Group B (63 cases)was given methotrexate intramuscularly or orally. The therapeutic effects of two groups were observed. Results The successful rate in group A and group B was 79 07%(68/86) and 96 82%(61/63). The successful rate of the patients with lower HCG level in group A and group B was 86 89%(53/61) and 100%(32/32) respectively, which had significant difference (P<0 05). Conclusion Conservative treatment was effective to the pharmaceutical incomplete abortion when the uterine bleeding was not severe. The method of suitable conservative treatment should be chosen according to the HCG levels.
