CT扫描对脊柱结核早期诊断的意义

目的:评价CT在脊柱结核早期诊断中的应用价值。材料与方法:10例早期脊柱结核患者,其中男性7例,女性3例,年龄为5～30岁,平均25岁,于发病后5个月至1年内经CT扫描后确诊。结果:CT扫描1、能清晰显示椎体的微小骨质破坏、死骨的大小与数目,以及死骨周围干酪样物的形成状况2、可清晰显示椎间盘的破坏程度、及相邻间盘及椎体软骨板的破坏状况,3、还可较早地发现双侧或单侧肿胀的腰大肌、腰小肌、髂腰肌及其脓肿等。CT确诊脊柱结核的中心型6例,边缘型3例,混合型1例。结论:CT可以显示X线上所不能显示的种种微小病变,它是脊柱结核早期诊断的重要工具。

Significance of CT Scan in the Early Diagnosis of Vertebral Column Tuberculosis

Purpose: To evaluate the significance of CT scan in the early diagnosis of vertebral column tuberculosis. Materials and Methods: Ten patients with vertebral column tuberculosis, in whom male were 7 cases, female were 3 cases, the age ranged between 5 to 30 years with a mean age of 25 years,all underwent CT scans within 5 months to one year after onset of clinical symptoms. Results: CT scan clearly revealed small osteo - destructive lesions of vertebral body, size and numbers of necrotic bone and formation of caseous substances in periphery, destructive extent of inter-vertebral discs and cartilagineus plates,except these,CT also revealed whether bilateral or unilateral psoas and iliacus were swelling or formation of abscess.CT accurately diagnosed 6 cases of central type,3 cases of border type, and 1 case of mixed type in all patients. Conclusion: CT can reveal various small lesions of Tb that to be invisible on X - ray plain film,therefore,it is an important tool in early diagnosis of vertebral column Tb.
