广州一起公园恙虫病聚集性事件的现场调查与应急处置

摘    要:目的研究广州一起公园恙虫病聚集性疫情的流行特征及防控策略和措施,为今后开展类似暴发疫情的预防和控制提供参考。方法运用现场流行病学调查方法对广州一起公园恙虫病聚集性暴发疫情进行调查,采用外斐氏反应和PCR方法对病例和动物标本进行检测。结果该起公园恙虫病聚集性疫情累计发病17例,死亡3例,病死率为17.65%;病例发病以发热、肺炎等呼吸道症状为主,身体某部位均有特异性焦痂;现场调查采集公园工作人员血清标本66份,外斐氏反应全部阴性,采集公园流浪猫血标本6份和游离恙螨105只,PCR检测结果亦均为阴性。结论可确认该公园内发生了恙虫病聚集性暴发疫情,感染地点可能与患者在公园内活动接触草坪有关。本次疫情现场防控对可能造成一定社会影响和引发公众关注的公共卫生事件提供了有效的防控经验和模式。

Abstract:Objective To explore the epidemic characteristics of the tsutsugamushi disease outbreaks in a park in Guangzhou,and to develop a model for the future prevention and control. Method Field epidemic investigation was applied and the samples were analyzed by PCR and Weil-Felix reaction. Results 17 cases,including 3 death cases, were reported in this park tsutsugamushi disease cluster event,with the mortality rate of 17.65%.The major symptoms were feverand pneumonia.The major cases had the activities history in XG park and specific eschars in the body. The results of Weil-Felix reaction from the 66 serum samples of the park worker were all negative. The results of PCR from 6 cat blood samples and 105 free chigger mite samples were also all negative. Conclusions The cluster event of tsutsugamushi disease in XG park was confirmed. The source of infection with the orientia tsutsugamushi appeared to be correlated with the activities of the workers in the lawn of the XG park.Results from this study provide an effective prevention measures for the control of public health events.
