远红外线热成像图在观察随意皮瓣活性中的应用研究

摘    要:目的:探讨远红外线热成像技术(IRT)在观察随意皮瓣活性中的应用价值。方法:10只SD鼠均刮除背部鼠毛,设计一个8cm×2.5cm蒂在尾部的随意皮瓣。切取后原位缝合,随后于术后0.5h、6h、24h、3天和7天应用远红外线热成像仪拍照,拍摄的热图通过热红外成像系统v5.3.2H进行分析。观察不同部位温度随时间变化的趋势,并将热图与大体标本对比。结果:在热图上,0.5h时整个皮瓣温度均较低,6h后近段皮温开始上升,并逐渐恢复至正常水平,而中、远段皮温仍较低。24h后,中段皮温开始升高,并在7天后达到正常水平。而远段皮温一直维持在较低水平。大体标本上,3天时可见皮瓣远端开始部分坏死,7天后边界清楚,平均坏死面积占皮瓣总面积32.17%。通过对比热图低温区和皮瓣坏死区,发现二者相似性很强。结论:应用IRT可以预测皮瓣的活性,在6h后皮温下降的区域,部分皮瓣将发生坏死,而在24h后皮温没有开始回升者,其皮瓣坏死的可能性很大。因此,应用IRT在术后6h后通过连续观察,可以推测皮瓣血运和后期坏死概率。

Abstract:Objective To assess the effect of infra-red thermography (IRT) system in minotoring the viability of random-pattern skin flaps. Methods 10 SD rats were used in the study,A dorsal random-pattern skin flap measuring 8cm×2.5cm was raised severally,the depicle of flap was deviced on the rump.The temperature of flap were measured in 0.5h,was 6h,24h,3d and 7d individually. Analying the temperature change of flap by the software IRT system v5.3.2H,To deduce the viability of flap combining with general sample. Results Analying the map of flap temperature, whole flap temperature keep falling in 0.5h;the temperature began recovery at the proximal portion while it were lower at middle and distal portions in 6h.The temperature at middle portion began increasing in 24h, and recovery in 7d. however,it was keep lower at distal portion.Observating general sample of the falp, the necrosis proportion of the flap was 32.17% ,that was similar to the lower proportion of temepatature map. Conclusion We could predict random-pattern skin flaps viability by analying the temepatature map of IRT.The lower tempreture region of flap would befall part necrosis if it can beaan increasina in 24h.
