腹腔镜肠粘连松解术22例治疗体会

目的探讨腹腔镜肠粘连松解术治疗粘连性肠梗阻的应用价值。方法回顾分析22例粘连性肠梗阻患者行腹腔镜肠粘连松解术的临床资料。结果 20例腹腔镜下手术成功,其中1例术中分破肠壁改为腹腔镜辅助下小切口肠粘连松解术。20例应用腹腔镜患者手术时间40-96 min,平均65 min,住院时间5-7 d,平均6.1 d,随访1-24个月无复发。2例中转开腹。结论腹腔镜肠粘连松解术在严格掌握手术适应证的前提下是安全可靠的,其具有患者创伤小、痛苦轻、瘢痕小、康复快、再粘连率低等优点,值得临床推广。

Experience in the application of laparoscopic enterolysis:A report of 22 cases

Objective To explore the clinical value of laparoscopic enterolysis for adhesive intestinal obstruction.Methods The retrospective analysis was carried out on 22 cases of adhesive intestinal obstruction hat underwent laparoscopic enterolysis.Results Among them,19 patients were treated by laparoscopic enterolysis,1 patient were converted to open surgery with microincision.The operation time ranged from 40 t o 96 minutes with an average of 65 minutes.The hospitalization was between 5 days and 7 days with an average of 6.1 days.The follow-up period is between 1 month and 24months.No intestinal obstruction recurred.2 patients were converted to open operation.Conclusions When indications are followed strictly,laparoscopic enterolysis is safe and reliable,which has many advantages such as minimal trauma,less pain,smaller scar,more rapid recovery and lower recurrences.
