瑞芬太尼复合丙泊酚麻醉应用于妇科腔镜手术临床探讨

目的探讨瑞芬太尼复合丙泊酚麻醉应用于妇科腔镜手术的临床效果及安全性。方法将84例妇科腔镜手术患者随机分为观察组和对照组,每组各42例,观察组采用瑞芬太尼复合丙泊酚全凭静脉维持麻醉,对照组采用芬太尼复合丙泊酚全凭静脉维持麻醉,比较两组患者的麻醉效果。结果观察组患者术后拔管时间、呼吸恢复时间以及睁眼时间均少于对照组,组间差异有统计学意义(P<0.05)。观察组术后3例患者出现恶心、呕吐不良反应,不良反应发生率为7.1%,对照组术后12例患者出现恶心、呕吐不良反应,不良反应发生率为28.6%,不良反应发生率组间差异有统计学意义(P<0.05)。结论瑞芬太尼复合丙泊酚全凭静脉维持麻醉具有操作简便、麻醉诱导快、术后苏醒快、不良反应发生率低等特点,是一种理想和安全的妇科腹腔镜手术麻醉方法,值得临床进一步推广使用。

Clinical discussion of remifentanil combined with propofol anesthesia in gynecologic laparoscopic operation

Objective To explore the clinical effect and safety of using remifentanil combined with propofol anesthesia in gynecological laparoscopic operation. Methods 84 cases of gynecological laparoscopic operation were randomly divided into observation group and control group,42 cases in each group,patients in observation group were treated with remifentanil combined with propofol anesthesia,patients in control group were treated with fentanyl combined with propofol anesthesia,compared the anesthesia effect of the two groups. Results Postoperative extubation time,respiratory recovery time and open time in observation group were less than that in the control group,the difference between two groups was statistically significant ( P < 0. 05) . 3 cases in the observation group had nausea,vomiting,the incidence of adverse reactions was 7. 1% ,12 cases in the control group had nausea,vomiting,the incidence of adverse reactions was 28. 6% ,the differences of incidence of adverse reactions between two groups was statistically significant ( P < 0. 05) . Conclusion Remifentanil combined with propofol anesthesia has the advantages of simple operation,rapid induction of anesthesia,postoperative recovery,low incidence of adverse reactions,so it is an ideal and safe anesthesia method in laparoscopic gynecologic operation,worthy of further clinical using.
